Updated Precertification Request Forms for Behavioral Health Services and Applied Behavior Analysis
MHD has updated the forms required for requesting precertification for Behavioral Health Services and Applied Behavior Analysis. The forms now contain an e-signature function. Providers can either print and wet-sign or insert their e-signature before faxing to the Behavioral Health Services Help Desk at (573) 635-6516. The forms can be found on the MO HealthNet Provider Forms page.
