TPL Insurance Forms Portal
The Missouri Medicaid Third-Party Liability (TPL) Unit has launched a new online portal for submitting TPL-1 (Insurance Information Request) and TPL-4 (Insurance Information Update) forms effective August 3rd, 2026.
Providers may use the portal to:
- Report new third-party liability (TPL) information for MO HealthNet participants.
- Submit updates or corrections to existing TPL information.
- Upload supporting documentation, if applicable.
The online portal provides a secure and efficient way to submit TPL information and helps ensure participant records remain current, supporting accurate claims processing.
Providers are encouraged to use the TPL Portal for future TPL-1 and TPL-4 submissions.
