Topical Fluoride Treatment

Topical fluoride treatment (code D1208) is a covered service for participants aged 20 and younger.

Topical fluoride treatment for participants 21 years of age and older in the comprehensive benefit package (pregnant, blind, and skilled nursing facility participants) is limited to the following conditions or criteria:
 

  • Participants with rampant or severe caries (decay)
  • Participants who are undergoing radiation therapy to the head and neck
  • Participants with diminished salivary flow
  • Intellectually disabled participants who cannot perform their own hygiene maintenance
  • Participants with cemental or root surface caries secondary to gingival recession
     

Topical fluoride treatment is not covered for adults in the limited benefit package.
