Structured Family Caregiving Waiver Required Daily Documentation Reminder

Structured Family Caregiving Waiver (SFCW) caregivers must adequately document on a daily basis the services provided to participants and submit this documentation to the provider agency to support that the services were rendered. Providers must submit this documentation to the State upon request. Failure to have documentation may result in recouped claims.

Daily documentation to support services rendered includes caregiver notes that record and track the following:

  • Participant’s status
  • Updates or significant changes in their health status or behaviors
  • Participation in community-based activities
  • Medication management records 
  • Other notable or reportable events

Although the SFCW does not require the use of an Electronic Visit Verification (EVV) system, an EVV system may be used to document services if the EVV records include the required caregiver notes. Refer to Electronic Visit Verification for more information on EVV.
