New Electronic Case Management System for Home and Community-Based Services (HCBS)
To meet the demands of an ever-growing HCBS population, the Division of Senior and Disability Services (DSDS) transitioned from CyberAccess to HCBS Fusion on May 5, 2025. This new system will support the entire lifecycle of HCBS, from intake and eligibility to assessment, person-centered care planning, service authorization, and other ancillary case management activities. The dynamic workflow support, scalability, and open architecture allow Fusion to grow alongside a growing aging population and ensure DSDS staff can efficiently navigate complex processes in one, consolidated digital workspace.
To educate HCBS providers, their staff, and other interested stakeholders on HCBS Fusion, reference materials have been posted to the HCBS Fusion site. Several training sessions covering HCBS Fusion features, navigation, module tours, how to submit referrals and requests, reviewing and accepting care plans, provider reassessments, and more are also posted on the site. The site also includes the HCBS Fusion Provider User Guide, access information, FAQs, memorandums, and more.
