What does Dual-Eligible mean?
Dual Eligible Special Needs Plans also known as D-SNP is a special type of Medicare Advantage Plan that provides health benefits to individuals who qualify for both Medicare and Medicaid. D-SNP’s offer more coverage and benefits than original Medicare. If you receive both Medicare and Medicaid benefits, this means you are dual eligible.

The Program of All-Inclusive Care for the Elderly (PACE)
The Program of All-Inclusive Care for the Elderly (PACE) is administered by MO HealthNet and Medicare to provide comprehensive health care, social, recreational, and wellness services to their participants. One of the main goals of PACE is to allow older adults to live safely in their homes instead of nursing facilities.
A PACE Organization is responsible for providing care that meets the needs of the participants in all aspects of life, 24 hours a day, every day of the year. All PACE centers include a primary care clinic and areas for participants to receive needed therapies, socialization, personal care, and dining services. With the help of an interdisciplinary team, the PACE center also serves as the focal point for the coordination and administration of most PACE services.

Telemedicine
Telemedicine is the delivery of health care services by means of information and communication technologies which facilitate the assessment, diagnosis, consultation, treatment, education, care management, and self-management of a patient's health care while such patient is at the originating site and the health care provider is at the distant site.
Telemedicine or telehealth can be especially valuable for rural participants, who may not have the provider or specialist they need close by.
